WinNT4.0 SP5, Voodoo3 2000, drvr 1.04.04. Poor text output at first, then I copied the file C:winntsystem323dfxVGL.dll (the OpenGL driver for V3) to the Catalysm Demo3DfxWinNT directory, and rename it to opengl32.dll (replacing the old file). The text output cleared up.
