Has anyone any suggestions, or is the card on its way out?

I think that's a known problem with creative geforces. Your fan needs some oil. If i'm not mistaken i read it a long time ago on www.geforcefaq.com

That is also the sign that the bushings or ball bearings on the fan are wearing out. I would suggest buying new heatsink and fan.

Check this web page out. The blue orb is the best after market heatsink/fan out there. It works with existing geforce cards.
